Community Skills Program® – A Division of Success Rehabilitation, Inc. offers a non-residential, post-acute rehabilitation program designed to help individuals with brain injuries transition from hospital or facility-based care to independent living in their own homes and communities.

Our dedicated staff provide one-on-one support in clients’ homes and communities, fostering self-directed, responsible behavior and promoting independence. Services are tailored to each individual’s needs, with flexible participation schedules that may include day programs or outpatient services.

Who is good fit for Community Skills Program? Adults over 18 who have experienced a brain injury and are looking for services in their home or community. Community Skills Program team members focused cognitive functions and strategies to help individuals get back to work and life.
Do you offer physical therapy, speech therapy, occupational therapy? No. We strongly value collaboration with an individual's neurorehabilitation outpatient team but no do not provide Physical Therapy, Speech Therapy, or Occupational Therapy.
What counties in Pennsylvania do you serve? Adams, Allegheny, Armstrong, Beaver, Berks, Bucks, Butler, Carbon, Chester, Cumberland, Dauphin, Delaware, Fayette, Lancaster, Lebanon, Lehigh, Montgomery, Northampton, Northumberland, Perry, Pike, Philadelphia, Schuylkill, Westmoreland, and York County.
What counties in New Jersey do you serve? Cumberland, Morris, Hunterdon, Warren, Sussex, Essex, Union, Somerset, Middlesex, Mercer, Monmouth, Bergen, Passiac, Atlantic, Cape May, and Ocean County.
What funding do you take? In Pennsylvania, Community Skills Program accepts funding from the Head Injury Program, Workers Comp, and Private Pay. In New Jersey, we accept the Horizon NJ through MLTSS, NJ TBI Fund, Workers Comp, and Private Pay.
Who does Community Skills Program work with? All of our clients have a primary diagnosis of a brain injury. We work with people ages 18- to 70 years old who have suffered a traumatic or acquired brain injury. This means we work with those who have suffered motor vehicle accidents, blunt force trauma to the head at work or in community, slip and falls, strokes, brain tumors, seizure disorders, and more.
Does it matter when the brain injury took place? No! Our brain injury rehab in Pennsylvania and New Jersey serves consumers who have suffered a recent brain injury, as well as those who have been dealing with the effects for years.
Do you offer Telehealth? Yes! Individuals can utilize Telehealth for appointments where they are not able to meet in-person for brain injury rehab for cognitive rehabilitation therapy. *Eligibility is dependent on the requirements of the funding course - call to learn more!

Our History
With over 40 years of experience, Community Skills Program® has a proud legacy of delivering exceptional brain injury rehabilitation services. Established in 1981 by Ray Finley, Ph.D., and Sally Kneipp, Ph.D., CRC, the program was designed to help individuals with traumatic brain injuries successfully reintegrate into their communities through personalized, one-on-one support in home and community settings. By the late 1980s, the program expanded its services across Pennsylvania and New Jersey.
In 2023, after more than four decades of dedicated leadership, Dr. Kneipp transitioned the Community Skills Program® to Success Rehabilitation, Inc. Stefani Eichelberger, M.S.W., LCSW, CBIS, CESP, a 14-year veteran of the program as client services liaison, now serves as director. Under the leadership of Joanne Tangney, M.S.W., LSW, president and CEO of Success Rehabilitation, the Community Skills Program® continues to provide innovative, person-centered rehabilitation services that promote independence and enhance quality of life for individuals with brain injuries.
